Please fill out this form with as much information as possible. This form is to be used for ONE SONG PER WRITER/COMPOSER. Complete your portion of the split as you see fit. Each person on the song will also complete this online split sheet. All parties will receive a PDF to be signed as long as all parties agree to the split amounts. This info is not shared between writers until the PDF is sent to each participate on the song.
